This toolkit is intended to give prescribers a broad overview of medication utilization management techniques and describe ways that prescribers can facilitate patient access to needed medications. It focuses on prescribed medications that are filled by retail pharmacies; it does not cover the processes for methadone or specialty pharmacy products, such as injectable or implantable medications, all of which are commonly covered by a patient’s medical benefit rather than their pharmacy benefit.

In this cross-sectional study of data from 3142 US counties, counties in the South Atlantic, Mountain, and East North Central divisions had more than twice the odds of being at high risk for opioid overdose mortality and lacking in capacity to deliver medications for opioid use disorder. Higher density of primary care clinicians, a younger population, micropolitan status, and lower rates of unemployment were associated with lower risk of opioid overdose and lower risk of lacking in capacity to deliver medications for opioid use disorder.

The consequences for failing to treat OUD within correctional settings are significant. In addition to risk of overdose, most individuals who receive MAT in the community lose access to treatment upon confinement; studies show these individuals experience extreme stress. Individuals with opioid dependence entering correctional facilities are also at high risk for opioid withdrawal syndrome.
