More than 600 people registered and 300 attended an information session on Thursday, June 30, 2022 about first-of-its kind funding to address rural homelessness from the U.S. Department of Housing and Urban Development. This presentation highlighted The Health Wagon’s Rural Communities Opioid Response program, Strengthening and Expanding Substance Use Disorder and Opioid Use Disorder Prevention, Treatment and Recovery Programs in Southwest Virginia. The program is a consortium-based implementation that aims to reduce the morbidity and mortality of substance use disorder (SUD), including opioid use disorder (OUD), in rural communities in Southwest Virginia at the highest risk for SUD.

With facts and figures about health status, behavioral risk factors, mortality, and access to care, the resource aims to inform rural health policy for four states – Arizona, California, New Mexico, and Texas – along the U.S. southern border. The chartbook is a collaboration between the FORHP-supported Rural & Minority Health Research Center and the National Rural Health Association.

This guide provides rural health care leaders and teams with foundational knowledge, strategies, and resources to understand the impact of social determinants of health (SDOH) on patients and communities. It organizes key information and resources to help the busy manager support and lead education and discussion with front-line staff. This guide focuses on (1) understanding the need and opportunity around addressing SDOH, (2) using local data to support decision making, and (3) involving team members to plan and implement action steps.

The chartbook is part of the annual National Healthcare Quality and Disparities Report (NHQDR) that assesses the performance of our health care system across these measures.
