In their new 2022 Clinical Practice Guideline, the Centers for Disease Control and Prevention (CDC) provides updated evidence and research on the risks and benefits of prescription opioids for acute, subacute, and chronic pain. These include dosing strategies, tapering and discontinuation, comparisons with nonopioid pain treatments, and risk mitigation strategies.

Innovative at their inception three decades ago, drug courts confront a practical and ethical obligation to reimagine some core practices and assumptions. A shifting legal and public health landscape means, for example, increased scrutiny of the courts’ focus on abstinence and mandated treatment, and the use of jail. In 2019, the Addictions, Drug & Alcohol Institute (ADAI) at the University of Washington launched the “Meds First” program to provide onsite, low-barrier access to buprenorphine in partnership with six syringe services programs across WA State. A key component of the Meds First service model was the addition of care navigation to support client engagement and retention in OUD treatment.

Telemedicine is increasingly being used to treat patients with opioid use disorder (OUD). It has particular value in rural areas of the United States impacted by the opioid crisis as these areas have a shortage of trained addiction medicine providers. Patient satisfaction significantly impacts positive clinical outcomes in OUD treatment and thus is of great clinical interest.

Whereas outpatient treatment with medication for opioid use disorder (MOUD) is evidence based, there is a large network of inpatient facilities in the US that are reimbursed by commercial insurers and do not typically offer MOUD. This study is a comparison of rates of overdose and hospitalization after initiation of medication for Opioid Use Disorder in the inpatient vs outpatient setting.
