The purpose of this document is to provide detailed guidelines of the Nurse Care Manager Model of Office Based Addiction Treatment program for management of substance use disorders, with particular emphasis on treatment of opioid use disorder with buprenorphine (alone and in combination with naloxone) and naltrexone (oral and extended-release injectable formulations).

Buprenorphine utilization is an effective treatment for opioid use disorder (OUD). Given the recent
increase in child maltreatment reports related to parental substance use, research should explore the correlation between buprenorphine treatment and child maltreatment–related outcomes.

In this cross-sectional study of 266 Medicaid MCO plans and 39 FFS programs, FFS programs offered more generous MOUD coverage, but a higher percentage of FFS programs imposed PA than MCO plans.

Providers Clinical Support System (PCSS), a Substance Abuse and Mental Health Services Administration-funded grant initiative, announced the release of Practice-Based Guidelines: Buprenorphine in the Age of Fentanyl. The guide gives practitioners a blueprint on treating fentanyl use disorder based on available research combined with emerging clinical experience on the use of buprenorphine in the treatment of individuals using fentanyl and other highly potent synthetic opioids.

Medications for opioid use disorder, including buprenorphine hydrochloride and methadone hydrochloride, are highly effective at improving outcomes for individuals with the disorder. For pregnant women, use of these medications also improves pregnancy outcomes, including the risk of preterm birth. Despite the known benefits of medications for opioid use disorder, many pregnant and nonpregnant women with the disorder are not receiving them.

The most effective therapy for people with opioid use disorder involves the use of Food and Drug Administration-approved medications—methadone, buprenorphine, and naltrexone. Despite evidence that this approach, known as medications for opioid use disorder, reduces relapse and saves lives, the vast majority of jails and prisons do not offer this treatment.
