This webinar will provide participants with an understanding of the role of the medical examiners and coroners in the surveillance of drug overdose deaths, as well as the current efforts to strengthen the death investigation system. The webinar will also cover an overview of the overdose fatality review process and tools available for implementation.

A disproportionate number of people in jails have substance use disorders (SUDs).1 Incarceration provides a valuable opportunity for identifying SUD and addressing withdrawal.* Within the first few hours and days of detainment, individuals who have suddenly stopped using alcohol, opioids, or other drugs may experience withdrawal symptoms, particularly when they have used the substances heavily or long-term. Without its identification and timely subsequent medical attention, withdrawal can lead to serious injury or death. Deaths from withdrawal are preventable, and jail administrators have a pressing responsibility to establish and implement withdrawal policy and protocols that will save lives and ensure legal compliance. This brief describes the scope of the challenge, provides an overview of constitutional rights and key legislation related to substance use withdrawal, and outlines steps for creating a comprehensive response to SUD.

Background: Sharp exacerbations of the US overdose crisis are linked to polysubstance use of synthetic compounds. Xylazine is a veterinary tranquilizer, long noted in the street opioid supply of Puerto Rico, and more recently Philadelphia. Yet its national trends, geographic distribution, and health risks are poorly characterized. Methods: In this sequential mixed-methods study, xylazine was increasingly observed by ethnographers in Philadelphia among drug-sellers and people who inject drugs (PWID). Subsequently, we systematically searched for records describing xylazine-present overdose mortality across the US and assessed time trends and overlap with other drugs

In 2019, the National Institutes of Health (NIH) began supporting research on treatment for opioid use disorder in criminal justice settings. The Justice Community Opioid Innovation Network (JCOIN) studies the effectiveness of new medications and other interventions as part of the NIH HEAL Initiative – Helping to End Addiction Long-Term.

Over the past 20 years, drug overdose deaths have increased dramatically in the United States. Most of these deaths involved opioids, including prescription pain medications, heroin, and synthetic opioids such as fentanyl. These are called opioid-related overdoses and often occur as a result of respiratory depression caused by opioids, even when other medications and drugs are involved.

Since 1999, an estimated 841,000 people in the U.S. have died from a drug overdose. Beginning in March 2020, the COVID-19 pandemic significantly exacerbated the overdose crisis resulting in a 30% increase in 2020 compared to 2019. The majority of overdose deaths in the U.S. involve opioids, including nearly 71% of all overdose deaths in 2019.3 Despite high rates of overdose across the nation, overdose and overdose death are preventable. However, people at risk of overdose often face significant challenges accessing treatment and navigating systems of care. Local and state health departments are well-suited to lead and support efforts to prevent and respond to overdose and to link people to evidence-based treatment and services. Peer support services (PSS) are a valuable component of a growing number of overdose response and linkage to care initiatives that can be implemented and supported by local and state health departments.

CDC’s Division of Overdose Prevention has developed an interactive data visualization tool, the DOSE Dashboard, which displays current nonfatal overdose data. The nonfatal drug overdose data on this dashboard are from CDC’s Drug Overdose Surveillance and Epidemiology (DOSE) system, which captures electronic health record information in syndromic surveillance data systems. DOSE data can be used to compare monthly and annual trends in nonfatal drug overdose-related ED visits by drug type (i.e., all drugs, all opioids, heroin, and all stimulants), state, and sex and age group.

The data visualization in the National Vital Statistics System, received and processed by the National Center for Health Statistics (NCHS), shows the 12 month provisional number and percent change of drug overdose deaths. The analysis dashboard also shows the provisional number of drug overdose deaths by drug or drug class.

The CDC’s National Vital Statistics System has been updated to now include data related to February 2020 overdose mortality, with trends indicating further worsening of the nation’s opioid epidemic.
