Project Summary
Our project is a collaborative effort with NMORC, MyMichigan Healthcare, and Munson Healthcare. This project incorporates telehealth services, telehub services, (including 24/7 telehealth support with CMH), and utilizes the screening tool SEEK-Safe Environment for Every Kid. The screening tool identifies social drivers of health and utilizes a navigator for support.
