Improve access to and utilization of prevention, treatment, and recovery services in Toombs County in order to improve the care for those affected by behavioral health conditions, which may include substance use and mental health disorders. Our target population includes: (1) individuals who are at-risk for SUD/OUD and/or co-occurring mental disorders; (2) individuals diagnosed with SUD/OUD and/or co-occurring mental disorders; (3) individuals in treatment and/or recovery for SUD/OUD and/or other co-occurring mental disorders; (4) their families and/or caregivers; and (5) impacted community members who reside in Toombs County (a HRSA-designated rural area).
