Project Summary
Coordination of care and discharge is provided across the Kolbe Clinic treatment teams. The RCORP-BHS project will add new support for patients through having a Peer Support Specialist available to support individuals receiving SUD and other behavioral health care services under this grant in identifying and addressing barriers to treatment and recovery.
