Building on Part I of the stigma webinar series and its introduction of a statewide collective impact model for addressing stigma, this webinar delivered the first part of the model that also served as its conceptual framework. This webinar introduced the stages of change and showed grantees how these apply to their target populations. We also discussed how those same principles applied to grantee engagement of community stakeholders and their openness to evidence-based practices that reduce morbidity and mortality related to SUD/OUD.

This webinar provided the foundational information necessary to plan and implement effective anti-stigma strategies. This webinar focused on the crucial role of the emergency department (ED) in recognizing and treating opioid use disorder (OUD) patients with evidence-based medications for addiction treatment. Gail D’Onofrio, MD, discussed her pioneering work in creating the evidence for initiating ED buprenorphine treatment with ED patients presenting with opioid use disorder. Overall, the opioid epidemic intertwined with the COVID-19 pandemic has greatly escalated the need to mitigate the morbidity and mortality associated with the rising rate of fentanyl use. Data supporting the use of buprenorphine in the ED setting as well as the consequences of not initiating treatment will be discussed. While the use of ED prescribed buprenorphine has increased, universal adoption has lagged. Barriers to implementation of ED buprenorphine were discussed as well as strategies to overcome these challenges. Components of successful integration of an ED program with community partnerships was outlined. Current research by emergency physicians regarding innovative strategies such as high-dose buprenorphine inductions and use of extended release 7-day formulation of buprenorphine will be discussed. Initiation buprenorphine effectively, reduces withdrawal symptoms, improves adherence to treatment, and saves lives.

In recent years, much attention in the U.S. has been focused on the opioid crisis, which was responsible for nearly 46,000 overdose deaths in 2018. This crisis initially began to accelerate in the early 2000s with a steady rise in the abuse of prescription pain medications, and beginning around 2010, opioid deaths increasingly involved heroin. As of 2013, the ready availability of potent synthetic opioids such as fentanyl ushered in a new era of rapidly increasing opioid overdose deaths, with the total number of opioid deaths doubling between 2013 and 2018. Deaths involving synthetic opioids have continued to rise very rapidly, even as involvement of commonly prescribed prescription opioids and heroin has leveled off recently.
