Background: Sharp exacerbations of the US overdose crisis are linked to polysubstance use of synthetic compounds. Xylazine is a veterinary tranquilizer, long noted in the street opioid supply of Puerto Rico, and more recently Philadelphia. Yet its national trends, geographic distribution, and health risks are poorly characterized. Methods: In this sequential mixed-methods study, xylazine was increasingly observed by ethnographers in Philadelphia among drug-sellers and people who inject drugs (PWID). On Wednesday, the U.S. Department of Health & Human Services announced nearly $15 million awarded to rural communities to continue addressing misuse of illegal and prescription drugs known as psychostimulants. These awards are part of the Rural Communities Opioid Response Program (RCORP), a multi-year initiative with $400 million invested since its start in 2018.

A new brief from the National Center for Health Statistics gives geographic detail on the latest increase in overdose death rates. Overall, urban counties had higher rates, but eight states – California, Connecticut, Maryland, New York, North Carolina, North Dakota, Vermont, and Virginia – had rates that were higher in rural counties.

This report provides recommendations for actions that state and local leaders can take immediately to increase evidence-based practices, decrease arbitrary determinations, and prevent overdose deaths. The report also provides concrete steps that will, in the longterm, help dismantle a siloed system of unequal access and disparities and move towards an integrated system that promotes restorative justice, where people and families are treated with dignity, and where addiction is treated as a health and wellness matter rather than one of moral failing or criminality.

Goals: Increase knowledge of harm reduction principles, strategies, and resources, increase knowledge of managed use, abstinence, and safer use to meet people who use drugs where they are at, provide a safe environment (plenaries and breakouts) to discuss licit and illicit drug use as a multi-faceted phenomenon requiring successful interventions and policies and increase knowledge of stigma as it relates to harm reduction principles and practices.

In spring 2021, pharmaceutical manufacturers and distributors notified syringe services programs (SSPs) and partners that there would be significant interruptions in the supply of injectable/intramuscular (IM) naloxone. Currently, production and distribution delays are expected to last until fall 2021. This will specifically affect SSPs because IM naloxone is the most affordable formulation, and therefore is most commonly utilized by programs that distribute large volumes of naloxone to reduce fatal overdose rates.

West Virginia leads the nation in overdose death rate and both counties where participants were interviewed for this study were considered to be at risk for an HIV outbreak among people who inject drugs. This study asked the question ‘What on-the-ground environmental factors among PWIH could be contributing to this overdose rate?

Staff from Arkansas Behavioral Health Integration Network shared lessons learned from their RCORP-Planning grant in gaining buy-in for harm reduction concepts and planning for harm reduction services in rural Arkansas.
