This guide provides rural health care leaders and teams with foundational knowledge, strategies, and resources to understand the impact of social determinants of health (SDOH) on patients and communities. It organizes key information and resources to help the busy manager support and lead education and discussion with front-line staff. This guide focuses on (1) understanding the need and opportunity around addressing SDOH, (2) using local data to support decision making, and (3) involving team members to plan and implement action steps.

This digital toolkit from the U.S. Department of Agriculture and the University of Kentucky’s Community and Economic Development Initiative includes examples of rural placemaking projects along with technical assistance providers, funders, and guides to resources.

This week, the federal agency that researches what makes health care safer, more affordable, higher quality, and accessible to all released its compendium of data and trends for rural populations. The chartbook is part of the annual National Healthcare Quality and Disparities Report (NHQDR) that assesses the performance of our health care system across these measures.

More than 600 people registered and 300 attended an information session on Thursday, June 30, 2022 about first-of-its kind funding to address rural homelessness from the U.S. Department of Housing and Urban Development. With facts and figures about health status, behavioral risk factors, mortality, and access to care, the resource aims to inform rural health policy for four states – Arizona, California, New Mexico, and Texas – along the U.S. southern border. The chartbook is a collaboration between the FORHP-supported Rural & Minority Health Research Center and the National Rural Health Association.

The information in this document was guided by the vision of the U.S. Department of Health and Human Services’ Office on Women’s Health and lessons learned from a 3-year reentry enhancement project conducted across 3 different reentry organizations. The participating pilot sites were the Resonance Center for Women, Inc., the College and Community Fellowship, and the Institute for Health and Recovery . Using the information compiled through this project, this guide was created by the Substance Abuse and Mental Health Services Administration’s GAINS Center for Behavioral Health and Justice Transformation.
