In the context of the trends in Oregon SUD metrics and the increasing infectious complications related to IDU in other jurisdictions and their implications for HIV and HCV transmission, we sought to 1) describe statewide trends in IDU-related serious bacterial infections (SBI) hospitalizations overall and by SBI type and drug use diagnosis, 2) assess IDU-related SBI diagnoses among individuals living with HIV and HCV, and 3) and determine the annual costs of IDU-related SBI overall and by SBI type.

In 2015, 33,091 persons in the United States died from an opioid-related drug overdose.The epidemic of opioid overdose deaths has led to expanding the use of naloxone in community settings by non–medically trained bystanders who are often people who use drugs (PWUD). Since 2013, illicitly manufactured fentanyl (IMF), fentanyl analogs, and other synthetic opioids have played an increasing role in overdose deaths in the United States. Illicitly manufactured fentanyl (IMF) prevalence has increased. However, there is uncertainty about naloxone dose(s) used by nonmedical bystanders to reverse opioid overdoses in the context of increasing IMF.

In this context, the overarching aim of this document is to inform and encourage governments, policy–makers, and other partners to take the necessary actions to implement evidence-based prevention strategies and treatment services for substance use disorders in order to provide everybody, girls as well as boys, and women as well as men, with the skills and opportunities to prevent the initiation of unhealthy behaviours and, in case of individuals who use drugs and suffer from drug use disorders, with the optimal support for improving their life circumstances.

In recent years, much attention in the U.S. has been focused on the opioid crisis, which was responsible for nearly 46,000 overdose deaths in 2018. This crisis initially began to accelerate in the early 2000s with a steady rise in the abuse of prescription pain medications, and beginning around 2010, opioid deaths increasingly involved heroin. As of 2013, the ready availability of potent synthetic opioids such as fentanyl ushered in a new era of rapidly increasing opioid overdose deaths, with the total number of opioid deaths doubling between 2013 and 2018. Deaths involving synthetic opioids have continued to rise very rapidly, even as involvement of commonly prescribed prescription opioids and heroin has leveled off recently.

In New York City (NYC), there were 2062 overdose fatalities in 2020, the deadliest year on record for NYC and the US. Fentanyl and its analogs were the most common substances involved in overdose deaths in NYC, present in 77% of such deaths in 2020. A characteristic of fentanyl-involved overdose is rapid onset of overdose symptoms; however, with timely administration of oxygen or naloxone, deaths can be averted.

County jails and community organizations across Michigan are teaming up on a new approach to distribute naloxone (Narcan®), the lifesaving medication used to reverse an opioid overdose, through the use of customized vending machines.

Syringe services programs (SSPs) remain highly effective, cost-saving interventions for the prevention of blood-borne infections among people who inject drugs. However, there have been restrictions regarding financial resources allocated to these programs, particularly in the US South. This study aimed to provide cost data regarding the implementation and first-year operations of an academic-based SSP utilizing fixed and mobile strategies, including the integration of onsite wound care.
