Telemedicine is increasingly being used to treat patients with opioid use disorder (OUD). It has particular value in rural areas of the United States impacted by the opioid crisis as these areas have a shortage of trained addiction medicine providers. Patient satisfaction significantly impacts positive clinical outcomes in OUD treatment and thus is of great clinical interest.

In recent years, much attention in the U.S. has been focused on the opioid crisis, which was responsible for nearly 46,000 overdose deaths in 2018. This crisis initially began to accelerate in the early 2000s with a steady rise in the abuse of prescription pain medications, and beginning around 2010, opioid deaths increasingly involved heroin. As of 2013, the ready availability of potent synthetic opioids such as fentanyl ushered in a new era of rapidly increasing opioid overdose deaths, with the total number of opioid deaths doubling between 2013 and 2018. Deaths involving synthetic opioids have continued to rise very rapidly, even as involvement of commonly prescribed prescription opioids and heroin has leveled off recently.

The study tested a care coordination model in six rural primary care sites during the peak of the COVID-19 pandemic, July 2020 to January 2021. Each clinic tracked patients with opioid use disorder (OUD) as they received medication treatment delivered by an external telemedicine provider.

The guide from the National Council for Mental Wellbeing aims to support harm reduction organizations operating in virtual environments and summarizes some of the strategies that harm reduction organizations have developed and found to be effective at maintaining connection while doing harm reduction work virtually. Harm reduction continued during the COVID-19 pandemic and organizations effectively changed the way services were delivered, primarily through telework.
