The investigation focused on one Indiana-based hospital system that included six hospitals located in cities and seven that had rural Critical Access Hospital designations.

The Cascade of Care model measures the quality of outcomes at each stage of treatment, from diagnosis to long-term maintenance, and was first proposed in 1998 as an approach to care for HIV/AIDS.
