This guide provides rural health care leaders and teams with foundational knowledge, strategies, and resources to understand the impact of social determinants of health (SDOH) on patients and communities. It organizes key information and resources to help the busy manager support and lead education and discussion with front-line staff. This guide focuses on (1) understanding the need and opportunity around addressing SDOH, (2) using local data to support decision making, and (3) involving team members to plan and implement action steps.

More than 600 people registered and 300 attended an information session on Thursday, June 30, 2022 about first-of-its kind funding to address rural homelessness from the U.S. Department of Housing and Urban Development. The University of Rochester Recovery Center of Excellence, one of three FORHP-supported Rural Centers of Excellence on Substance Use Disorder, has developed comprehensive training on the treatment of opioid use disorder (OUD) in primary care. This no-cost training prepares providers and staff to deliver evidence-based care to patients. Continuing education credits are available.

Hepatitis C: State of Medicaid Access is a collaboration between the Center for Health Law and Policy Innovation of Harvard
Law School (CHLPI) and the National Viral Hepatitis Roundtable (NVHR). The project evaluates hepatitis C (HCV)
treatment restriction policies across state Medicaid programs, including all 50 states, the District of Columbia, and Puerto
Rico (referred to in this report as “states”). The goal of this project is to encourage states and empower advocates to work
toward improved access to curative HCV treatment across all Medicaid programs, in order to ensure that every Medicaid
enrollee who can benefit from this treatment is able to access it.

The Centers for Disease Control and Prevention and the National Organization of State Offices of Rural Health have joined with two philanthropic organizations with experience in minority communities – Well-being and Equity (WE) in the World and Well Being In the Nation (WIN) Network – to collaborate on guidelines for what it will take to bring health equity to rural areas.

This study links parental incarceration to adolescent substance use in rural communities. The data is state-specific and pulls from the 2019 Minnesota Student Survey in which adolescents self-reported experiences. Results concluded that parental incarceration was associated with higher substance use and expanded prevention and intervention strategies for adolescents could help reduce the rates.

Corporation for Supportive Housing (CSH) developed a Racial Disparities and Disproportionality Index that looks at 16 unique systems and measures whether a racial and/or ethnic group’s representation in a particular public system is proportionate to, over or below their representation in the overall population and also allows for the examination of systematic differences between groups and geographies .
