People suffering from addiction or people who are in recovery from the condition, face a variety of challenges, including, in many cases, in their interactions with health-care services. Many of these challenges may be attributed to the stigma that still clings unhelpfully to addiction. It may also be due to a surprising lack of awareness even among health-care professionals about the nature of addiction and the susceptibilities and anxieties of those initiating or attempting to sustain recovery.
This review and report is a starting point, an attempt to get a handle on the state of play and the issues that need addressing when it comes to pain management in people with current or past addictions. It helps to identify gaps in knowledge, understanding, skill, practice and culture, pointing the way to how these deficiencies might be remedied.

State policymakers are interested in learning about the most effective treatments to address OUD and avoid unintended consequences such as overdose events, mortality from overdose, and use of illicit and unregulated drugs. Syringe service programs (SSPs), which provide access to sterile syringes and other injection equipment and their safe disposal after use,* represent a highly successful human immunodeficiency virus (HIV) prevention intervention. SSPs are associated with a 58% reduction in the incidence of HIV infection among persons who inject drugs. In addition, SSPs have led efforts to prevent opioid overdose deaths by integrating evidence-based opioid overdose education and naloxone distribution (OEND) programs. OEND programs train laypersons to respond during overdose events and provide access to naloxone and directions for drug delivery. SSPs are ideal places for OEND because they provide culturally relevant services designed to reach persons at high risk for experiencing or observing an opioid overdose.
