Since 1999, an estimated 841,000 people in the U.S. have died from a drug overdose. Beginning in March 2020, the COVID-19 pandemic significantly exacerbated the overdose crisis resulting in a 30% increase in 2020 compared to 2019. The majority of overdose deaths in the U.S. involve opioids, including nearly 71% of all overdose deaths in 2019.3 Despite high rates of overdose across the nation, overdose and overdose death are preventable. However, people at risk of overdose often face significant challenges accessing treatment and navigating systems of care. Local and state health departments are well-suited to lead and support efforts to prevent and respond to overdose and to link people to evidence-based treatment and services. Peer support services (PSS) are a valuable component of a growing number of overdose response and linkage to care initiatives that can be implemented and supported by local and state health departments.

Substance use and its progression to impairment, loss of control, and development of substance use disorders is a significant problem for adolescents and young adults. The suggested tools and resources herein address the need for agencies to support frontline service providers following exposure to an overdose fatality. Frontline service providers may include any staff members that interact directly with populations that are at greater risk for experiencing overdose. The suggestions throughout the document should be implemented in keeping with agency-based policies and procedures for staff support. Introducing these strategies into the workplace can foster greater well-being among those staff members most vulnerable to trauma and distress.
The goal of these suggestions is to promote well-being in the workplace for those most vulnerable to trauma, stress and grief. To achieve this, outlined below are principles for agencies to incorporate into their organization, how to acknowledge death in the moment, approaches to coping with strong emotions, the importance of building a support system, and the process of grief.

This Guide on Prevention of Drug Use and Treatment of Drug Use Disorder in Rural Settings was prepared by the United Nations Office on Drugs and Crime Drug Prevention and Health Branch, in the context of the global project Treatnet II: OFID-UNODC Programme to prevent HIV/AIDS through Treatnet Phase II, with the aim of providing an awareness-raising tool and guidance for policymakers, public health officials, local authorities and other stakeholders in dealing with substance use issues in rural settings in their respective countries.

In 2016, the United States Congress authorized and appropriated funding to fight the opioid epidemic through the Department of Health and Human Services . Among other initiatives, the Comprehensive Addiction and Recovery Act (CARA) gave the Substance Abuse and Mental Health Services Administration authority and support to implement new grant programs focused on preventing overdose and treating individuals with OUD.
