We know from RCORP grantees, first responders, hospitals, people who use drugs, the media and other allies that many communities are seeing spikes in overdose (OD) events and deaths since the onset of COVID-19. In 2015, 33,091 persons in the United States died from an opioid-related drug overdose. The epidemic of opioid overdose deaths has led to expanding the use of naloxone in community settings by non–medically trained bystanders who are often people who use drugs (PWUD). Since 2013, illicitly manufactured fentanyl (IMF), fentanyl analogs, and other synthetic opioids have played an increasing role in overdose deaths in the United States. Illicitly manufactured fentanyl (IMF) prevalence has increased. However, there is uncertainty about naloxone dose(s) used by nonmedical bystanders to reverse opioid overdoses in the context of increasing IMF.

In New York City (NYC), there were 2062 overdose fatalities in 2020, the deadliest year on record for NYC and the US. Fentanyl and its analogs were the most common substances involved in overdose deaths in NYC, present in 77% of such deaths in 2020. A characteristic of fentanyl-involved overdose is rapid onset of overdose symptoms; however, with timely administration of oxygen or naloxone, deaths can be averted.
