Project Summary
The Safe Communities Coalition will focus on serving individuals who are at risk for, have been diagnosed with, and/or are in treatment and/or recovery for psychostimulant disorders; their families and/or caregivers; and chronically marginalized and underserved populations, including but not limited to: Racial and ethnic minorities; low-income populations; pregnant women; transition age youth; and people experiencing homelessness. The Safe Communities consortium will expand the Safe Rx coalition to include a focus on education, prevention, intervention, treatment and recovery of the most prevalent psychostimulant substance use issue in Lake County, methamphetamine. In addition, the Safe Communities coalition will provide harm reduction and trauma-informed trainings to educate stakeholders; expand capacity to offer and sustain culturally responsive services; distribute educational material throughout the community to assist in de-stigmatization of service access; establish cross-sector data sharing with partner agencies; enhance discharge coordination for people in treatment and recovery; expand peer workforce; and community and target populations to learn about the Safe Rx and the Safe Communities Coalition project. The Safe Communities Coalition will continue to bridge the gap between social determinants of health and other systemic issues, by collaboratively addressing health challenges of residents and serving communities with impactful solutions that establish linkages, promote and expand recovery communities, leverage shared resources, and coordinate care. The Safe Communities Coalition will leverage partnerships at the local/community, state, and regional levels, such as with rural counties and municipalities, tribal governments, various agencies providing healthcare coverage, law enforcement, community recovery organizations, faith-based organizations to expand and streamline the continuum of care for the region.
