Hope for a Drug Free Stephens Corporation

Project Summary

Hope for a Drug Free Stephens Consortium members have partnered to create an integrated behavioral health network called the Stephens County Wellness and Recovery Network. This program will serve individuals who are at risk for, have been diagnosed with, and/or are in treatment and/or recovery for psychostimulant use disorders; and their families and/or caregivers in Stephens County. The Network will deploy a value-based, person-centered approach with an emphasis on care coordination and treating the whole patient by integrating primary care with substance use and mental health treatment and recovery services both in-person and via telehealth.
