Project Summary

Our Pathways to Recovery Team works in conjunction with the many programs within and outside of our agency to connect clients to resources necessary to get and stay substance-free while maintaining their independence and personal preferences for doing so. The Pathways Overdose Team (PORT) project will target gaps identified during the course of providing services through other grant projects to address community problems associated with the opioid epidemic. The needs to be addressed are increased capacity and access to treatment, including MAT providers, through telehealth services, filling gaps in our existing QRT services, reduction of barriers to treatment, and increased access to transit to treatment. The target population for this project is (1)individuals at risk for, have been diagnosed with, and/or in treatment and /or recovery for OUD,(2)their family members/caregivers, and (3)the community as a whole.
