Project Summary

The goal of the UP MOM Model Expansion Program is to improve the quality of care for pregnant and postpartum Medicaid beneficiaries with opioid use disorder residing in Luce, Mackinac, Alger, Schoolcraft, Menominee, Dickinson and Iron Counties by implementing Community Health Worker (CHW) centered care coordination strategies that focus on cross-system collaboration and improved health outcomes over the course of the three year performance period.
