•Establish new MAT access point in Powell, Sheridan, Cody and via Mobile Unit CMHD counties. •Partnership with Powell Drug. •First MAT patient within 6 months. •MAT access points with in-person and virtual encounters with medical, BH, CBHPSS, CHW/PRS, and access to medications to treat SUC/AUD •In-person care using centering groups •All BH PSS certify as Recovery Doulas •2 CBHPSS supported to train to become LAC's (Clinically trained licensed provider with specialized training in SUD) •1 OH employee will be supported to gain a degree in BH role •Recruit CBHPSS (mentor 5 people) •Convene Advisory Meetings •Designated Learning Collaborative Point of Contact to facilitate monthly OH staff meeting to discuss strategies •Connect patients with PRS/CHW and enroll in telehealth platform •Ensure LAC visit and CD eval and Treatment plan •Facilitate licensed clinicians to obtain licensure in both Montana and Wyoming to expand pool of telehealth-ready providers
