Project Summary

BRIDGES - Building Resources Impacting Determinants Gaining Essential Services, BRIDGES is a proactive response to data-driven and informed, demonstrated community need for a coordinated, integrative, continuum of support service and resources for justice involved individuals and other target populations. BRIDGES is focused on the outreach, access, prevention, wellness, and recovery opportunities for rural residents who are experiencing chronic illnesses, poor health outcomes, isolation, shortened life span, behavioral health disorders, and are otherwise disconnected from community connections. Each of the targeted sub-populations experience and the rural population as a whole identify barriers to care and lack of health insurance as significant concerns and priorities.
