Jersey Community Hospital District

Jersey Community Hospital (JCH), a public hospital district located in Jerseyville, Illinois, as part of the Community Addiction Recovery and Education Services (CARES) Consortium requests $1,000,000 to implement a three-year R-CORP Implementation 2020 project to implement core prevention, treatment, recovery, and additional services for residents of Greene County and two rural census tracts of Jersey County, Illinois. The overall goal of the CARES Implementation project is to reduce the morbidity and mortality associated with substance use disorder (SUD), including opioid use disorder (OUD), in high-risk rural communities by utilizing the collaborative relationships and well-established organizational and infrastructural capacity of the CARES Consortium (CC) to address prevention, treatment, and recovery needs in Greene County and two rural census tracts in Jersey County, Illinois.
