Project Summary
Southern Healthcare Collaboration, Inc. and the Consortium will enhance safety net features for youth and families throughout Appling and Bacon counties, addressing needs for additional providers, additional supportive services, additional prevention and awareness, additional training opportunities, and enhanced community coordination to better protect youth. Regular mental health and SUD screenings will be offered in schools, and after school care programs will be established. Naloxone will be distributed, and trainings offered for staff and families. Bacon and Appling Counties will add a Psychiatric Mental Health Nurse Practitioner, a Healthcare Navigator, School-Based Counselor, Behavioral Health Training Specialist, two ABA Therapist / Registered Behavior Technicians, and a Transportation Coordinator. Peer parent support programs will be established for parents. Relevant providers in the region will become Certified Mental Health Technicians. Local colleges will send 30 students annually to complete Rural Health Rotations at pediatric and family practice locations in Appling and Bacon Counties. Peer Mentor groups will be established for paraprofessionals, youth development professionals, and community members, with training in Youth Mental Health First Aid and Naloxone distribution. Transportation and daycare services will be provided to support families so parents can attend appointments with their children. After school programming will be recovery friendly to support positive youth development. Families will be connected with fresh produce. The Healthcare Navigator will work with community providers to enhance discharge coordination from emergency departments and juvenile detention centers to ensure smooth transitions to community-based care.
