Through our affiliated rural hospitals and practices, we serve rural and Appalachian communities of Upstate NY with a full range of primary through quaternary health care services. The greatest challenges we face in expanding access to MAT are provider-based stigma, reimbursement, primary care practice support, and integrating behavioral health back into primary care.

UConnectCare

to reduce (1) the morbidity and mortality of substance use disorder (SUD) (including opioid use disorder (OUD)) and (2) the incidence and impact of Neonatal Abstinence Syndrome (NAS) in Genesee and Orleans Counties by improving systems of care, family supports, and social determinants of health. NAS is a group of conditions that can occur when newborns withdraw from certain substances, including opioids, that they were exposed to before birth. Our project goals are to: 1.

Farnham, Inc.

Farnham Inc. is a not-for-profit agency providing solutions to the problems created by substance use with an uncompromising commitment to quality and professionalism. Farnham will lead the project with partnering agencies Desens House & Cayuga County Community Mental Health Center. The Coordinated Overdose Response Project will support immediate response to the overdose crisis in rural areas of Oswego and Cayuga counties through improving access to, capacity for and sustainability of prevention, treatment, and recovery services for substance use disorders (SUD).

RCORP-Rural Centers of Excellence Showcase Webinar Materials

This webinar will introduce the newest RCORP program cohorts to the RCOEs, Fletcher Group, University of Rochester, and University of Vermont. These three cooperative agreements are charged with supporting the identification, translation, dissemination, and implementation of evidence-based programs and best practices, in addition to providing specialized TA to RCORP recipients and other rural providers.

Westchester-Ellenville Hospital

This project will allow a consortium serving five counties in New York State, to implement an innovative protocol for hospitals, emergency medical services, providers of medication assisted treatment, and other treatment and recovery providers, to collaboratively engage patients with opioid overdose immediately and intensively, through treatment and recovery.

Oswego County Opportunities, Inc.

OCO and the Oswego County Opioid Response Consortium members will strengthen and expand SUD/OUD prevention, treatment, and recovery services for pregnant/parenting women, youth/young adults, the homeless population, LGBTQ individuals, and persons involved in the criminal justice system in Oswego County. Particular focus will be on increasing awareness of the disparities and reducing barriers to increase access to services for all individuals.

Rochester Institute of Technology

The Priority Behavioral Health Team at CHST utilizes a Digital Health Suite that includes Telehealth, Simulation Technologies, Digital Therapies, 3-D Modeling, and evidence-based treatments that are personalized and easily accessible to co-occurring mental health/SUD clients, their families, and the community.

Oswego County Opportunities, Inc.

The goal of this initiative is to reduce the morbidity and mortality of SUD/OUD, particularly related to psychostimulant misuse in high-risk rural communities by strengthening and expanding prevention, treatment and recovery services. OCO will work closely with the other members of the Oswego County Opioid Response Consortium to complete the core activities in order to enhance individuals’ ability to access prevention, treatment, and recovery services.

Margaretville Memorial Hospital

The overall objective of the Catskills Rural Communities Opioid Response Program is to provide evidence-based, harm reduction-focused services and education to individuals living in rural communities who are at risk for or diagnosed with SUD/OUD, and their caregivers/families (target rural population), through leveraging existing resources and expanding capacity in the areas of prevention, treatment, and recovery.
