Researchers looked at what happens in rural and urban emergency departments (EDs) when peer-based services are used for patients arriving with opioid use disorder (OUD). The study aimed to find gaps in knowledge for rural EDs and found five key differences from urban counterparts that presented a challenge. Among these was difficulty identifying community partners.

The North Carolina Emergency Department (ED) Peer Support Program enables participating North Carolina hospitals to embed certified peer support specialists in their emergency departments to connect patients presenting with opioid overdose to treatment, recovery, resources and harm reduction supports. Drug overdose is a nationwide epidemic that claimed the lives of over 100,000 people in the United States in the past year. Opioids, either alone or in combination with other drugs or alcohol, were responsible for approximately 70 percent of these deaths. Many of those 70,000 people would be alive today if they had been administered the opioid antagonist naloxone and, where needed, other emergency care.

The pandemic landscape has exacerbated the magnitude of the opioid crisis and challenged our health care infrastructure’s ability to respond. More than 93,000 people died from an overdose in 2020, 30 percent more than the previous year and the highest number since the federal government declared a public health emergency in 2017. Fueled by a rise in non-prescription opioids, namely heroin and illicit fentanyl, 2021 is on track to be the deadliest year on record for the opioid crisis in the United States.

This webinar focused on the crucial role of the emergency department (ED) in recognizing and treating opioid use disorder (OUD) patients with evidence-based medications for addiction treatment. Gail D’Onofrio, MD, discussed her pioneering work in creating the evidence for initiating ED buprenorphine treatment with ED patients presenting with opioid use disorder. Overall, the opioid epidemic intertwined with the COVID-19 pandemic has greatly escalated the need to mitigate the morbidity and mortality associated with the rising rate of fentanyl use. Data supporting the use of buprenorphine in the ED setting as well as the consequences of not initiating treatment will be discussed. While the use of ED prescribed buprenorphine has increased, universal adoption has lagged. Barriers to implementation of ED buprenorphine were discussed as well as strategies to overcome these challenges. Components of successful integration of an ED program with community partnerships was outlined. Current research by emergency physicians regarding innovative strategies such as high-dose buprenorphine inductions and use of extended release 7-day formulation of buprenorphine will be discussed. Initiation buprenorphine effectively, reduces withdrawal symptoms, improves adherence to treatment, and saves lives.

Background: Racial/ethnic disparities in the use of opioids to treat pain disorders have been previously reported in the emergency department (ED). Further research is needed to better evaluate the impact race/ethnicity may have on the use of opioids in adolescents for the management of pain disorders in the ED.

The position of the American College of Medical Toxicology (ACMT) and American Academy of Clinical Toxicology (AACT), is as follows: Fentanyl and its analogs are potent opioid receptor agonists, but the risk of clinically significant exposure to emergency responders is extremely low. To date, we have not seen reports of emergency responders developing signs or symptoms consistent with opioid toxicity from incidental contact with opioids. Incidental dermal absorption is unlikely to cause opioid toxicity
