The consequences for failing to treat OUD within correctional settings are significant. In addition to risk of overdose, most individuals who receive MAT in the community lose access to treatment upon confinement; studies show these individuals experience extreme stress. At least 95 percent of individuals in state prisons will eventually return to communities. In fact, in a typical year more than half a million people do so, with many more coming from jails. A disproportionate share of these individuals have one or more chronic illnesses, including more than half who met the criteria for a non-alcohol and nicotine-related substance use disorder from 2007 to 2009, according to the latest available data.

The nation’s opioid epidemic continues to compromise the health and well-being of individuals, families, and communities. Federal policymakers and agencies are developing, implementing, and funding strategies focused on turning the tide to address opioid misuse, opioid use disorder, fatal and non-fatal drug overdoses, prenatal substance exposure, dissolution or breakup of families, and financial ruin experienced in communities nationwide.

The information in this document was guided by the vision of the U.S. Department of Health and Human Services’ Office on Women’s Health and lessons learned from a 3-year reentry enhancement project conducted across 3 different reentry organizations. The participating pilot sites were the Resonance Center for Women, Inc., the College and Community Fellowship, and the Institute for Health and Recovery . Using the information compiled through this project, this guide was created by the Substance Abuse and Mental Health Services Administration’s GAINS Center for Behavioral Health and Justice Transformation.
